In most cases, the facility will have to be in-network to be covered. It also cannot be a luxury or private facility.

The level of coverage provided for inpatient rehab services and costs vary depending on the plan. Because Humana requires patients to get referred to a treatment facility before they will pay, it is critical that patients review their treatment options with their physician before deciding on treatment. Although Humana only covers in-network facilities, it does not differentiate based on geography, meaning that patients do not have to stay local to be covered.
